74 year old male, previously diagnosed with osteoporosis and was told by specialists and surgeons not to lift more than 5kg for the rest of his life... how could you possibly enjoy your life being that weak!?

Can you lift with a disc bulge?

One of the most common conditions we see in clinic, probably about 15 cases of these a week. Disc bulge or disc herniation (also inaccurately known as a "slipped disc") occurs when the soft inner material of a spinal disc bulges out through a tear in the tough outer layer, potentially causing pressure on nearby nerves and resulting in extreme pain, weakness, or numbness in the affected area.

I'm sure you or someone you know has had something like this before, and know how debilitating this thing can be. As much as it sounds like it only happens to people who aren't careful, about 80-90% of the cases are triggered whilst sedentary, or the popular "slept funny" or "lifted a pen with wrong technique". Which tells us that disc herniation are usually a build-up problem, where for over a prolonged period, your low back has been overworking and stressed, causing wear and tear through the disc, and eventually bulge out with even the smallest trigger.

So never blame the trigger, blame the cause. Find out what us causing the low back to work extra hard, because if you don't find that, you'll commonly see people suffering for years and decades until eventually they are told to get spinal fusion surgery. As shown in the video, it only takes a few days, maximum 1 week to get out of pain and back to lifting to build a strong foundation to the spine.

We had to share this video! This lady is such an inspiration to us all, and to us chiros, she is the epitome of the best patient we could ask for ! Here's why:

Elderly patients and parents come to us initially so frustrated (or even worse, given up) because they have no idea why they keep having pain, have no direction at all how to fix it, and the doctor just keeps telling them to take pain killers until eventually you need to get surgery (which only ruins you more long term)

Well the simple and harsh truth and root cause of all issues are, they're probably weak. And rightfully so, because back then, our parents didn't have proper education about strength and conditioning and less access to health facilities like gyms and chiros. Everyone was too busy surviving, ain't no time to lift weights.

However, if you have never lifted weights for years, haven't done any resistance training and all they do is "walk", then how do you counteract your aging and weakening muscles over time ? Well that's their problem and solution. The only way to fix your aging parents arthritis, bulging discs and back problems is to lift heavy!

This is the ultimate end goal. If you can get your mum so strong and robust enough to lift 130-150kg or squat 100kg, do you really think she would be having back pain picking up your grandchildren ? Or have knee problems walking down the stairs ?

When can you return to sports after an injury? The million dollar question for all athletes and gym enthusiasts! 🧐

Trust us, we share the same goal as you. We were or are athletes ourselves, so we know exactly how you feel. We don't intend to stop or slow your progress in returning to your sport, but sometimes, temporarily halting your activities is the quickest way to get you back. What's the point of pushing through pain and only being able to exert 50-60% of your capacity for months or years, risking further injuries, when you can treat it and return to playing at 💯% in a few weeks?

In this case, we have a superstar football and hockey athlete who had a debilitating knee injury for months, experiencing pain while doing all the things she loves, like hiking, squatting, football, and hockey. The problem, however, was that she kept doing activities that were causing pain and made no progress.

After negotiation and compromise, we reached a deal that would temporarily stop her from engaging in painful activities, giving us the opportunity to heal and rehabilitate the knee. Just being pain-free is not good enough for athletes; they need to be pain-free IN THEIR SPORT. She was having pain with any single leg loading movements, which there are alot it when changing directions in soccer and hockey. She was cleared to play once we knew the knee was capable of handling movements and loading on the painful knee.

That is mainly because the low back is overloaded or "working harder" to compensate for a weaker or dysfunctional surrounding. IS KNEE CAVING IN BAD !?

When we watch some of the best lifters in the world, especially the world record holders, they all have something in common. Their knee caves in! It is almost a natural movement to recruit more power generating muscles like the adductors instantaneously coming out of the bottom of the squat. We all do it, and it's a natural technique when lifting something heavy. We gotta stop hating on it!

Still to date, too many people, coaches and influencers are promoting and encouraging lifters to push your knees out and NEVER LET YOUR KNEE CAVE IN! We've heard it so many times, especially the classic cause knee pain in heavy squats is due to their "weak VMO" and their knee buckles inwards. 🤡

However, we do think there are a couple important points:

1. Knee caving in shouldn't be happening with body weight or significantly lighter weight squats as you shouldn't be needing to recruit this technique for such a low demand task. This could indicate certain mobility or stability issues around the lower body.

2. We don't encourage people to purposely train this knee caving in technique. Your body will naturally do it when it's a heavy weight, just stop thinking that it's such a bad thing to do and NO it will not blow your ACL

3. If your knee hurts doing squats, theres a million reasons, but your technique might not be one. So stop trying to fix your technique when it can be a very simple solution somewhere else

TRICKING YOUR OWN BRAIN is a hard thing to do but a powerful method, especially in the rehab world.

After injuries or medical conditions, controlling the affected site becomes tougher due to pain, weakness, or atrophy. No matter how hard you try to move or hold something, if the brain is resisting, executing tasks becomes nearly impossible.

In this case, the common peroneal nerve in the left leg is compromised, causing the left foot to struggle with dorsiflexion (bringing the toes up towards you). Even when instructed to lift both simultaneously, the brain's resistance hinders the movement on the left foot.

But here's the game-changer: a mirror in the middle tricks the brain into perceiving movement in the left leg. The result? He genuinely believes his left leg is working normally. This powerful visual feedback system provides crucial neural information to the compromised nerve and muscle function, making the rehab process smoother and efficient.

He will have to practice this at home with a mirror quite often to continue getting that healthy feedback to his brain and regain control of his left foot.
